The Medical Service Store accepts faxed orders as well as orders entered through this site. If you have received a promotion code or discount code, these can also be used on faxed orders. Since most Purchase Orders are automatically generated, you can hand write the promotion code on either the PO or the cover sheet.  When you use your promotion code, we’ll automatically register you on our site, so that you can receive order confirmation and shipment information. Please include an email address with your order.

 

If you currently have an account with Carestream Health Inc., we’ll add you account information to your order when it’s processed. If you don’t have an account with Carestream Health Inc., a credit card is required to process the order.
